Continuity of Care Maturity Model (CCMM)

 

A strategic framework to guide continuity of care implementation

Created to help optimise outcomes for health system and patient alike, the Continuity of Care Maturity Model (CCMM) outlines the progressive capabilities healthcare organisations need to possess in order to seamlessly coordinate patient care across multiple care sites.

The model addresses the convergence of interoperability, information exchange, care coordination, patient engagement and analytics with the ultimate goal of holistic individual and population health management. It reports a care system’s maturity in eight stages - stage 0: low maturity to Stage 7: high maturity.
